New! Advance Care Planning Certificate
End-of-life planning is a sensitive topic and social workers are ideally suited to initiate and guide these discussions as they explore an individual’s values and preferences. In fact, a reported 96 percent of clinical social workers conduct advance care planning conversations (ACP) and 80 percent are responsible for educating patients and families about ACP options.
NASW-DE and NASW-NJ, in partnership with Goals of Care Coalition of NJ, has developed a certificate program for those new to these conversations and for seasoned professionals who would like a deeper understanding of the issue.
The three-part program includes: 2 CEU hours of pre-foundational reading; 4 CEU hours On-Demand videos and culminates in 4 CEU hours of live instruction (September 30, 2020, 9AM-1PM).

Course Highlights
All parts of the program must be completed to receive your certificate
Part One: 2 Clinical CEU hours of pre-foundational reading, including articles from
- a) Clinical Social Work Journal
- b) Institute for HealthCare Improvement
Part Two: 4 (3 Clinical; One Social/Cultural) CEU hours of On-Demand videos, including:
- a) The Basics of Advance Directives
- b) Fundamental Principles of Serious Illness Communication.
Part Three: 4 Clinical CEU hours of live instruction, including:
- a) Application of Best Practices - participants will take part in moderated role play
- b) NJ POLST (Practitioners Orders for Life Sustaining Treatment) – detailed instruction on the importance of this form and how to complete it.
- c) Opportunities for Social Workers to Lead – building and maintaining an organization-wide ACP program
