Advance Care Planning Certificate
End-of-life planning is a sensitive topic and social workers are ideally suited to initiate and guide these discussions as they explore an individual’s values and preferences. In fact, a reported 96 percent of clinical social workers conduct advance care planning conversations (ACP) and 80 percent are responsible for educating patients and families about ACP options.
NASW-NJ, in partnership with Goals of Care Coalition of NJ, has developed a certificate program for those new to these conversations and for seasoned professionals who would like a deeper understanding of the issue. This is the only certificate ACP course in NJ!
The three-part program includes: 2 CEU hours of pre-foundational reading; 4 CEU hours On-Demand videos and culminates in 4 CEU hours of live instruction (June 11, 2021, 9AM-1PM).

All parts of the program must be completed to receive your certificate
Part One: 2 Clinical CEU hours of pre-foundational reading, including articles from
a) Clinical Social Work Journal
b) Institute for HealthCare Improvement.
Part Two: 4 (3 Clinical; One Social/Cultural) CEU hours of On-Demand videos, including:
a) The Basics of Advance Directives
b) Fundamental Principles of Serious Illness Communication.
Breaking Barriers in End-of-Life Care
Facilitating Effective Goals of Care Conversations
Part Three: 4 Clinical CEU hours of live instruction, including:
a) Deep Dive into Settings: Acute, Long Term, Community, and Private (breakout rooms by setting)
Ap NJ POLST (Practitioners Orders for Life Sustaining Treatment) – detailed instruction on the importance of this form and how to complete it.
Opportunities for Social Workers to Lead - building and maintaining an organization-wide ACP program
