End-of-life planning is a sensitive topic and social workers are ideally suited to initiate and guide these discussions as they explore an individual’s values and preferences. In fact, a reported 96 percent of clinical social workers conduct goals of care conversations and 80 percent are responsible for educating patients and families about their options. 

In this webinar, Gabrielle Winther, LCSW, will present on social workers role in goals of care conversations.  Tips and techniques for these sensitive conversations, including active listening, will be explored as well as how clients and families can self-advocate within the medical system.  This workshop will examine how to navigate when clients and families have different ideas about palliative objectives.  Gabby will discuss how past traumas can impact decision-making and how that relates to the medical decision making process.

2-Identify ways for pts to use psychosocial healthcare history to identify goals of care

3-Identify tools to utilize trauma informed care during goals of care conversations

Gabrielle Winther is a graduate of Rutgers University, School of Social Work with a concentration in Gerontology. Gabrielle has worked in case management, hospice and oncology and currently works in Palliative care at Hunterdon Medical Center. As a clinical social worker in Palliative Care, Gabrielle is part of a multidisciplinary team working with chronically ill patients and their families to address goals of care and provide supportive counseling. Gabrielle works with patients both in the hospital setting and in the outpatient office.
