Join an end-of-life expert from Hospice of the Northwest for a presentation about Advance Care Planning.
Advance Care Planning ensures your goals, values and preferences are known, not only at the end of life, but if you become unable to communicate those desires due to illness or injury. Participants will learn about:
- The importance of advance directives in making your wishes known - Choosing a healthcare agent or Durable Power of Attorney for Healthcare (DPOAHC) - What a POLST form is and why you might need one
