Web-based Certified Quality Auditor Refresher Course
ASQ Section 0509 is pleased to offer an online refresher course that provides quality professionals with foundational quality auditing concepts, tools, and techniques; and prepares him/her to take the American Society for Quality’s (ASQ) Certified Quality Auditor (CQA) exam. The course is designed to provide foundational quality auditing information through pre-reading materials, on-line recordings, and live webinars, using Quality Council of Indiana’s Certified Quality Auditor Primer.
Format:
Web-based, online course using Anymeeting.com online meeting platform. Participants will need access to the internet. Use of a headset is strongly encouraged. The new course syllabus and expectations is available

Expectations:
1) Watch on-line recordings and read Primer assignments prior to live webinars.
2) Take weekly quizzes and provide answers to instructor at least 24-hours prior to live webinars.
3) During live webinars: Take a pre- and post-poll and participate by posting and responding to fellow participants’ questions during webinars.
4) Have access to internet, and computer/device to be able to view live webinars real-time.
Dates and Time:
The course consists of pre-recorded sessions and live webinars. The webinars runs for 5 consecutive Mondays from July 30th through Monday, August 31st for 2-hour sessions. Each webinar will be held from 6:00 PM -8:00 EST. All sessions will be recorded and available for viewing to participants for 30-days.

Instructor: Sandra L. Menzies, MS, ASQ-CQA, ASQ-CQM/OE, PMI-PMP
This is the seventh year, Ms. Menzies has taught the CQA Refresher course for ASQ Section 509. Ms. Menzies has been a senior member of ASQ since 2001. With over 20 years of experience, Ms. Menzies has held quality-related positions at Biocon, Inc.; Otsuka Maryland Research Institute; TherImmune Research Corporation; and the Food and Drug Administration (FDA). Ms. Menzies current position uses her knowledge of auditing and project management to improve internal business processes at the FDA. In addition to auditing for compliance to Good Clinical Practice and Good Laboratory Practice regulations and ISO 9001 and ISO 17025 standards, Ms. Menzies has hosted FDA and EPA inspections, and ISO audits.
Course Materials: We will be using certification materials from the Quality Council of Indiana. Required materials for the course are:
+ CQA Primer, 8th edition (Jun, 2012) - $70
+ SolutionText - $35
+ CQA Exam CD - $70, optional yet recommended.
Notes:
1) We recommend that you purchase print version of the Primer, since the electronic version is not allowed to be taken into the test center.
2) The solutions text and CD-ROM from the Quality Council of Indiana are optional yet highly recommended.
3) Costs do not include tax, shipping and handling. Students should order the materials directly from the

ASQ CQA Exam Cost: $498 for non-members, $348 for members. Join ASQ and save! Exam applicants are responsible for meeting ASQs certification requirements.

Course Withdrawal Policy:
The policy for the Web-based CQA course starting July 30, 2018: If you need to cancel your registration or withdraw from the course, we will refund your paid registration fee as follows:
1. Requests for cancellations/withdrawal received by July 16, 2018, receive a full refund.
2. Requests received between July 17 and July 30, 2018, will incurs a 25% processing fee.
3. There are no refunds past August 6, 2018.
