The Annual Health Check (AHC) training session is designed for clinicians who have an interest in, or provide support to, individuals with a learning disability.
This session aims to provide participants with additional knowledge about the importance of the AHC. It will cover topics such as navigating the AHC template on EMIS, exemplifying best practices, and offering tips for delivering a high-quality AHC for patients with learning disabilities. The training will also include case studies illustrating strategies to enhance medication adherence.
Moreover, we will be joined by nurses from the community learning disability team who will delve into their roles and functions. They will present additional case studies showcasing how primary and community care collaborate to benefit our learning disability community.
By the end of this session, attendees will have the ability to effectively conduct AHCs in their practice and gain an understanding of which teams or disciplines they should refer to for the care of their LD patients.
