Aim:
To introduce you to the models, strategies and approaches to health promotion and explore the health inequalities that currently exist. We then introduce you to the elements of the NHS Health Check (touching on the Vital 5 Mental Health Check) for patients. This will provide you with the knowledge and support to begin to undertake the NHS Health Check independently, understand the results and be able to give brief advice to your patients.
Learning outcomes:
By the end of the session you will:
- Be able to identify the reasons why these checks are offered.
- Be familiar with the procedures and tools to undertake the NHS Health Check.
- Understand the ways we can prevent and educate patients to protect them from the risk of disease.
- Recognise the models and strategies used in health promotion.
- Identify some of the health promotion strategies in relation to current health priorities.
- Have had an opportunity to explore how the theories can be applied in practice.
- Develop an awareness of factors that impact on the success of health promotion activities.
- Understand what motivational interviewing and Making Every Contact Count is, and how these can be used to improve outcomes for patients.
- Be prepared for a period of supervision in practice to become competent in undertaking the NHS Health check.
Please Note: This session is not to be undertaken instead of the Vital 5 Check training if this is delivered in your practice; this course provides some additional learning to help you develop the confidence to have a brief conversation with your patient about their mental health whilst they are attending for their Health Check, and signpost if required.
