IT WILL BE $175 PER ATHLETE PER WEEK

Waiver and Informed Consent Statement :
In consideration of registering for participation in the activities of NUMA-Speed LLC. Camp, I agree to allow images taken today to be used in any photo , video, or online source for the purposes of marketing the program or ownership thereto. I also herby affirm to be medically able to participate in the activities offered by NUMA-Speed LLC.. I understand that there are risks that may include disabling injury and/or death involved in all activities involved in all physical activites, and I agree to familiarize myself with all equipment,rules and physical demands related to the activites undertaken. I agree to and waive any and all liability of NUMA-Speed LLC., and its respective officers,employees, members,volunterrs, and sponsors, and do hereby for myself heirs, executors and administrators waive and release and forever discharge any and all rights and claims for damages which I may have or which may accrue to me arisisng out of or in any of the activites of the NUMA-Speed Camp. I ackowlege the particular hazard and potential danger involved in allowing my participation in the camp. I give my persmission to medical staff to secure a licensed physician in the case of an emergency in order to provide the necessary care.
