Before you can participate in this psychoeducational workshop, the law requires that we obtain your informed consent. You can only provide us with your informed consent after we have discussed your proposed services, the potential risks of those services, the potential benefits of those services, and information about any possible alternative services. Please tell us immediately if you experience any health changes or ever become uncomfortable during the workshop.
I, the undersigned participant, acknowledge that Soleil Psychotherapy, its owners, agents, or employees may provide psychoeducational services to me.
I acknowledge that the ideas, goals, and methods of the psychoeducational workshop have been explained to me, and my facilitators will use procedures that are appropriate for the group.
The expected outcome of the workshop:
• Increased knowledge and understanding of a particular topic or issue related to mental health or well-being
• Improved coping skills and strategies for managing symptoms or stress
• Increased awareness of personal strengths and resources
The most common risks of participating in the workshop include, but are not limited to:
• Your condition may be unchanged or worsen, or a new problem may develop
• You may be at risk for injury to yourself or others
• Exposure to triggering or distressing content
Alternate treatment modalities include individual therapy, self-help/psychotherapy groups, environmental changes, inpatient services, and community resources, including churches, social services, the criminal justice system, and other governmental agencies.
I understand that this consent can be withdrawn from me at any time and that I am entitled to a copy of this consent. I further acknowledge that:
The psychoeducation group and its risks, benefits, side effects, and alternatives have been explained to me;
The therapy services may not have the result that I expect, and I have been informed as to other possible treatments that may provide me a benefit;
I have not been given any guarantees about the result of any treatment, and I have had ample opportunity and time to discuss my concerns with the facilitators, therapist, or any healthcare provider that I desire and all my questions have been answered to my satisfaction.
By signing below, I acknowledge that I am competent, understand this policy, and have been provided material information regarding the proposed care, treatment, service, intervention, or procedure, and the anticipated risks, benefits, side effects, and alternatives, as well as the risks of non-treatment. Thus, I hereby provide my informed consent to receive therapy services.
